HEMS Clearance Packet - ONLINE
No more paper clearance packets!
Students must complete the online Athletic Clearance Process (including a physical performed by a MD, DO, PA, or NP) at least one day before tryouts/practice/games for all competitive sports.
Use these INSTRUCTIONS to help you with the online process.
How to sign up:
- Visit your doctor for a sports physical (very similar to a well child visit - if done recently, your doctor can likely complete the form)
- 2024 - 2025 Athletic Physical Form
- this form needs to be printed and the doctor's office with stamp the form to make it official
- once complete you'll take a picture and upload it into the online clearance portal
- Physical MUST be completed May 1, 2024 or later. Physicals completed prior to this date will not be accepted.
- Complete the ONLINE CLEARANCE
- Go to https://www.homecampus.com/login
- Sign in
- In the top right choose "Start Clearance here" and follow the steps.
- Enter: EDDY MS (when choosing a school)
- Make sure to upload your physical into the attachments
- Once clearance packet is approved, your student is cleared for ALL sports at Eddy
- if pieces are missing, you will receive an email on what needs to be fixed for official clearance

- Check the HEMS Calendar for try-out dates and game/tournament information
- Due to limited players, Volleyball and Basketball teams hold try-outs. Tryouts happen over the course of 3 days and athletes are expected to attend all three days. Typically 12-18 students will fill the roster for the team. Even if your student does not make the team, tryouts are a great way for students to put themselves out there, meet new friends with similar interests, and have some fun.
A new online Athletic Clearance is needed every academic year.
